All returning and incoming athletes including Cheer and Dance team members, must complete the Athletic Clearance process before they can participate in any summer and/or school-year sports activities.
The Athletic Clearance/Physical is valid for one year and must be repeated on an annual basis. Failure to complete the Athletic Clearance/Physical prior to summer will result in the student not being able to try out or participate in any summer rec and/or school-year sports/activities at Tustin High School. Participation is only allowed after the Athletic Clearance has been approved by Tustin High School's Athletic Trainer, Joe Ferguson.

For Participants in Non-Compulsory District Activities
All student athletes, including students involved in Band, Cheer and Dance, must complete this form prior their first day of practice. A parent or guardian must sign the form if the participant is a minor. All completed waiver forms must be scanned and emailed to our Trainer, Joe Ferguson at email@example.com and a hard copy should be brought to our Athletic Secretary, Therese Hogue located in the Athletics Office 607 once school resumes in person.
Athletic Clearance/Physicals must be on file before a student will be allowed to try out for a sport/activity. A current Athletic Clearance/Physical is required for each school year and is valid for one calendar year from the date it was signed by the doctor. All medical information will be kept confidential according to HIPPA regulations.
