When you are offered a job, complete your portion of the form. Have the employer fill out their part and have your parent/guardian sign the form.
Please bring the completed Intent to Employ form to the Mr. Case during lunch in the PE Office located at the Event Center facing the Basketball courts to obtain the official work permit.
