Transcript requests can be made in person or by mail.
If you are requesting a transcript or student records in person or via mail, please utilize THIS FORM.
Valid identification will be required for all student records.
If anyone other than the student is picking up the transcript, the former student must authorize in writing the release of the record(s) and indicate to whom the record(s) is to be released. A valid photo identification of the student must accompany the receiver of the record(s).
If transcripts are to be mailed to you or to schools, please make certain that the receiving address is correct and accurate.
